Prescreening & Assessment for Mental Health Sponsored Residential
Prior to or at admission, the provider shall complete an initial assessment sufficient to determine whether the individual meets admission criteria and whether the provider has the capability and staffing necessary to provide appropriate services. The assessment shall evaluate immediate service, health, and safety needs. At a minimum, the assessment shall address:
- Diagnosis;
- Presenting needs;
- Psychiatric needs;
- Support needs;
- Onset and duration of identified concerns;
- Current medical problems;
- Current medications;
- Current and past substance use;
- Co-occurring disorders;
- Risk of harm to self;
- Risk of harm to others; and
- Other factors that may affect the individual’s safety or appropriateness for placement.
Additional information shall be obtained when necessary to determine whether the proposed sponsored residential home is an appropriate placement.
